In order to reduce the number of last minute cancellations and no shows, the following scheduling fee & cancellation policy must be accepted before you can schedule a consultation appointment with me. Please note that I have many patients with serious conditions who requires my attentions and care on daily basis. Please respect their needs by keeping your appointment, and do not schedule an appointment until you are absolutely sure that you can commit to the appointment. Non-member follow up consultation fee Non-member follow up consultation fee I understand that as a Non-Member, I am NOT entitled to special $25.00 consultation fee. Instead there is a non-member follow up consultation fee of $60.00 payable before the visit.

Non-Member Rescheduling Policy & Fee Non-Member Rescheduling Policy & Fee As a non-member, I understand that if I reschedule my appointment, there will be a minimum rescheduling fee of $20.00 regardless when the notice was given. However, if I request to reschedule my appointment 72 hours or less leading up to my original scheduled appointment time and date, there will be no refund. And I understand that I will be asked to pay $60.00 to schedule another appointment.

Non-member Cancellation Policy & Fee Non-member Cancellation Policy & Fee If I cancel my appointment, I understand that there will be a minimum cancellation fee of $20.00 ($40.00 refund) regardless of when the notice was given. However, if I cancel my appointment within 72 hours or less leading up to original appointment time and date, there will be no refund. And there will be no refund for NO-SHOW appointment.
