In order to reduce the number of last minute cancellations and no shows, the following scheduling & cancellation policy must be accepted before you can schedule a consultation appointment with me. Please note that I have many patients with serious conditions who requires my attentions and care on daily basis. Please respect their needs by keeping your appointment, and do not schedule an appointment until you are absolutely sure that you can commit to the appointment. Initial consultation fee Initial consultation fee I understand there is an Initial consultation fee of $129.00 payable at the time of scheduling. The initial consultation fee is non-refundable after the consultation.

Rescheduling policy & fee Rescheduling policy & fee (72 hours or less): If I request to reschedule my initial consultation within 72 hours leading up to the appointment time, I understand that I will be asked to pay additional $60.00 for rescheduling the initial consultation appointment. (More than 72 hours): If request to reschedule more than 72 hours prior to the appointment time, I understand that I will be asked to pay additional $30.00 (instead of $60.00) for rescheduling the initial consultation appointment.

Cancellation Policy & Fee Cancellation Policy & Fee (72 hours or less): If I request to cancel my initial consultation within 72 hours leading up to the appointment time, I understand that there will be a cancellation fee of $60.00 ($69.00 refund). (More than 72 hours): If I request to cancel my initial consultation more than 72 hours prior to the appointment time, there will be a cancellation fee of $30.00 ($99.00 refund). And there will be no refund for NO-SHOW appointment.
